Additional insurance participation is currently in progress. Please contact the office to verify coverage. Coverage may vary depending on specific plan details. Clients are encouraged to verify benefits prior to the first appointment.
Self-Pay
If we are not in network with your insurance plan, self-pay services are available. Fees will be discussed prior to scheduling to ensure clarity and transparency.
Out of Network
For clients using out-of-network benefits, we can provide a superbill upon request for potential reimbursement. Reimbursement is determined by your insurance carrier.
Payment and Cancellation Policy
Payment is due at the time of service unless prior arrangements have been made. Appointments cancelled with less than 24 hours’ notice may be subject to a cancellation fee.

At Creating Pathways, LLC, we are committed to providing thorough and accurate clinical care. At times, patients may request completion of forms, letters, or documentation for employers, schools, disability claims, or other third parties.
These requests require clinical review, assessment, and professional judgment outside of scheduled therapy sessions.
Policy Guidelines:
Requests for forms or letters must be submitted in advance and include all required documentation.
Completion time is typically 5–10 business days, depending on complexity.
Expedited requests may be accommodated based on availability and may incur additional fees.
All documentation must be clinically appropriate and supported by the patient’s treatment record.
Professional Fees:
Because these services are completed outside of session time and are not typically covered by insurance, the following fees apply:
Brief letters or simple forms: $50
Standard forms: $75
Complex forms (FMLA, disability, workplace accommodations): $125–$150
Comprehensive disability or multi-page evaluations: $150–$250
Payment is required prior to completion of the requested documentation.
Please note that submission of a request does not guarantee approval of the documentation, as all forms must reflect accurate clinical findings.
